I always heard that the exchange rate in airport is not good. If you are in Tokyo, just look at stores specialisez in money exchange in Tokyo (I change Yen to Won in a store close to Okachimachi station, there is some store there that can exchange), ask people in the state to tell you the rate (or check online if you want to do it with a know bank or exchange company) and compare with Narita rates : http://www.narita-airport.or.jp/exchange_e/

It is not possible to give a definitive answer. Also depend if you have other payment method when you get in the states.

We always exchange money at Narita airport. It's safe and the rates are pretty reasonable unless you are exchanging a lot of money.

I think the rate is bad in Narita. I would change just a minimum.
